A bulging disc is not necessarily a cause for panic as they are fairly common in both younger and older people.   • Annulus- the tough outer portion that contains the nucleus (the dough)

With a disc bulge the material within a disc will remain intact with the annulus pushing out beyond its boundaries. In an herniation, the jelly (nucleus) actually extrudes through the annulus. This is typically considered more severe and a greater cause for concern.

The bulging or herniated disc can cause leg pain or foot pain, known as sciatica, when it presses against (impinges) the sciatic nerve. If nerve compression is left untreated, it can result in permanent leg pain and weakness.

When a disc bulges enough to cause narrowing of the spinal canal it is also considered to be a serious condition. If there are bone spurs present on the facet joints behind the bulging disc the combination of these spurs and the bulging of the disc may cause further narrowing of the spinal canal in that area.

The care of a patient with a lumbar bulging disc is far from standardized. To a certain extend the treatment of a patient's bulging disc should be more individualized. Common therapies for lumbar disc bulges include chiropractic, physical therapy, lumbar traction, inversion table therapy and lumbar epidural steroid injections. The goal of therapy is to decrease the disc bulge and its compressive effect on the nerve root. Treatment of a bulging disc will mainly be dependent on the degree of bulge and/or nerve compression (as evidenced by MRI or CT Scan), the length of time the patient has suffered with his or her symptoms and the severity of the related pain. Most treatments will start with six to twelve weeks of conservative treatment.

Non-surgical spinal decompression can provide pain relief from a disc bulge or disc herniation. Spinal Decompression creates a negative pressure or a vacuum inside the disc. This effect causes the bulge or herniation to "regress" or draw back from the nerve it may be pressing against and the increase in negative pressure also causes the flow of blood and nutrients back into the disc thus facilitating the body's natural healing response. An example of this intervention is the flexion-distraction therapy which is utilized to great effect by your chiropractor in Chicago. A decade's worth of study has demonstrated this pain-free, non-invasive regimen to be approximately 80% effective - eliminating the need for drugs or surgery at all.

The amount of time invested in conservative treatment needs to be addressed on an individual patient basis. No patient's symptoms will present alike. In the patients that are still able to lead a productive lifestyle with only minor aches and pains a longer conservative treatment would most likely be advised. Back surgery for bulging disc treatment should be the absolute last resort. For some patients the pain and loss of productivity is just too much to handle and after conservative treatments are explored for a shorter duration and there are no changes in the patient's symptoms, surgery to decompress the affected nerve is advised. Of course, patients that do not feel any recovery after six to twelve weeks of conservative care will often find themselves looking into the final bulging disc treatment option of surgery for their bulging disc.

NOTE: Any patient who has progressive neurological deficits, or develops the sudden onset of bowel or bladder dysfunction, should have an immediate surgical evaluation as these conditions may represent a surgical emergency. Fortunately, both of these conditions are very rare, and most surgery for a lumbar herniated disk is an elective procedure.

Exposing The Myths, Untruths and Outright Lies About Chiropractic


Considering the increasing amount of people suffering from health problems and chronic pain in the United States, it's amazing to find how few have considered chiropractic care as a potential solution. If they could only get past the negative and misleading stereotypes, a majority would benefit greatly from some form of chiropractic care. Most people don't get or seek the care they need because they and the people they talk to, are victims of the myths, untruths and outright lies about alternative medicine. This article is meant to set the record straight and expose some of the most common misconceptions.

Looking Beyond The Stereotype of Chiropractic

Somewhere along the line, you may have heard the phrase, "Cracking Bones doesn't do anything for you." This is a great example of a completely untrue and negative stereotype of chiropractic. What chiropractors actually do, is adjust your spine. In Lehman's term's, when spinal bones get subtly out of alignment. they pinch or choke the delicate nerves as they exit the spine. These nerves control the function of every part of your body. Symptoms like musculoskeletal pain arise from misalignment of the spine. An adjustment treats these symptoms and provides pain relief, by providing a subtle correction of your spine's alignment. Adjustments also restore the normal, proper function of your nerves and entire body. "Cracking Bones" doesn't do anything for you, but getting a proper adjustment makes you healthier and acts as a natural pain reliever.

Let's Look At Some of The Top Myth's, Untruths and Outright Lies

#1 - Chiropractors are not real doctors

Overuse Injuries - What Causes Them and How to Avoid Them - Part 1


This is the time of year all athletes should be working on improving technique and becoming more fit and balanced. Sports injuries are the result of forces that become too much for bone or soft tissue to absorb and adapt too and therefore cause failure. All sports injuries are the result from an acute or sudden force that is too much for the tissue to overcome (e.g. a sprained ankle or broken bone) or from repetitive forces that overtime create so much stress that the tissues finally fail partially or completely.

These are most commonly known as overuse injuries or repetitive motion injuries. It is these types of injuries that most parents, coaches and athletes do not understand or know how to prevent. For this reason these are the types of injuries that we will focus our preventative techniques on.

Three categories or causes of overuse injuries:

1) Increased training load/volume
2) Inadequate warm-up (this actually results in an acute type of injury)
3) Biomechanical imbalances/improper technique

In Part one of this three part series we will discuss increased training load or volume as a cause for overuse injury.

Increased training load or volume occurs in early season when runners go from not doing much running or very little running outside. The switch to outside with the nicer weather often leads to a rapid increase in training volume. In addition, running outside requires that our legs do more work to turnover due to the friction of the road and the fact that it doesn't move under us like a treadmill belt.

What is the best way to go about preventing too rapid of a training load or volume? Be smart and follow the following guidelines. Make sure that activity increases occur in small increments of no more than 10 percent increase per week in total volume. If you have been running a total of 10 miles a week than you should not increase to more than 11 miles the following week. Using periodization for workouts with a 3 week build phase and then a fall back week in volume also helps prevent overuse injuries. Total volume of running, biking, or lifting is important, but one must also consider intensity.

So when doing speed work for example, first make sure that you are challenging yourself with an appropriate pace. If you are going out to do quarter mile intervals at 5k pace, make sure that you have raced a 5k recently so you know what pace you should be attempting to run at. Running at to slow a pace will not allow for fitness gains. Running at too rapid a pace for the intervals will result in inability to complete the workout and/or too much trainign stress and subsequent overuse injury risk. Once you have determined the appropriate intensity for your intervals you must also consider the volume (total time of each interval and or distance).

Again, make sure not to increase the number of intervals or the total time/distance of all the intervals by much more than 10% per week. Remember fitness gains occur during the rest period after we have caused muscle fatigue and breakdown. If you fail to give your body adequate rest and recovery time, including putting in the proper recovery fluids and nutrients you leave yourself at higher risk of an overuse injury.

The main benefit of sales at the point of service, commonly called "point of sale" (POS), is improved patient relationship. By offering your patients quality products that enrich their lives outside of your office, you demonstrate your care. Your patients are reminded of you - and of your care about them - every time they use the products you sell them. Better patient loyalty means lower attrition, frequent referrals and, eventually, improved profits.

Inventory Management

Chiropractic Care and Bed Wetting


Bed wetting is a common problem for millions of children and pre-teens around the world. Clinically known as "nocturnal enuresis," bed wetting can be as embarrassing as it is annoying. People try all sorts of methods to try to eliminate the problem. And yet there may be an easier way to cure it than you might think. One new therapy involves children visiting a chiropractor for spinal manipulations.

Why Do Kids Wet the Bed?
It's all about the phrenic reflex. Here's what happens to the body during sleep: While sleeping, breathing slows down until the body's carbon dioxide levels rise high enough to trigger a sudden shudder. This shudder is the phrenic reflex, which is also a spinal reflex. Then one takes a nice deep breath, and then starts breathing normally again.
If the phrenic reflex is not functioning properly, however, instead of triggering a shudder, high carbon dioxide levels in the body will cause the smooth muscles to relax. These involuntary muscles include the valve at the end of the urinary bladder. In this case, any fluid in the bladder is released, and the bed is wet.

How Can Chiropractors Help?
If the phrenic reflex is simply underdeveloped or not yet matured, time may be the only help. Yet often the reflex is fully developed, it is just obstructed by a vertebral misalignment known as subluxation. This out-of-place vertebrae puts pressure on the spine and keeps it from correctly reacting to the normal sleeping processes.

Chiropractic care can help determine whether or not your child has a misalignment, and potentially help your child conquer bed-wetting for good by manipulating the spine back into the proper position.

Does It Really Work?
There have been several studies conducted on this connection. One study, published in the Journal of Manipulative and Physiological Therapeutics, examined 171 children who typically wet the bed every single night. Each child was given some chiropractic care to realign any problems in the spine.

After the treatment, the average "wet" nights for the entire group shrank from seven to four. And 25 percent of the participants displayed a 50 percent reduction in bed-wetting nights. Another 15 percent were basically cured of their problem after the treatment, being considered "dry" with no nightly accidents.

A similar study, used a control group of bed-wetting children that were not treated with chiropractic care. Of the 46 children in the study, 31 received treatment and this group showed an almost 18 percent reduction in wet nights, while the children in the untreated group showed no improvement at all.

So while several visits to the chiropractor may not totally solve the problem, they can lessen the nightly leaking. Many children see marked improvement after just one or two treatments, but for others the results may be more noticeable after several weeks or months of adjustments.

So even if your pediatrician cannot help you solve your child's bed-wetting problem, don't despair. You can turn to chiropractic care to solve this embarrassing problem and get some much needed relief.

Chiropractors and How To Find a Good One


When you say the word "chiropractors", you may get one of several reactions. It is true that there are many who have no use for the profession and consider it a quasi-science at best. Others believe it is just the alternative they need to overcome serious back problems. Are You Thinking of Opening Multiple Chiropractic Practices?


You have been in practice for many years. You have been successful in building and maintaining your practice. The professional goals you established upon graduation have been far exceeded. A common business plan is to open a second office and work in it on your days off. Build a small patient base and then an associate can take over the existing patent base. What invariably occurs is the existing practice decreases in profitability, a small practice is built at the satellite practice, overhead costs almost double, and the doctor is working an additional two days, in addition to the regular work week, with zero financial ability to hire an associate. The doctor now finds him or herself bleeding until they hemorrhage. The reality is every hour they spend at the satellite office they are losing money and enthusiasm.

This failed plan plays over and over. I have benefited by purchasing many of my satellite practices from doctors at the hemorrhage stage. The doctors all felt that selling their practices at an undervalued price is better than continuing to loose money and energy. For them, it was to sell or close the office completely.

The fundamental mistake these doctors have made is they have worked in their practice, not on their practice. This statement by Michael Gerber in the E-Myth must be understood completely before attempting any expansion.

Doctors tell me all of the time that they are working harder then they ever have before, spending time traveling from one practice to another, training new staff, and seeing more people. They are not accomplishing their goal of multiple practices; they are just doing more of what they're already doing with a lot more overhead. They are working in their practice.

The Chiropractor whose goal is multiple practices must work less as a Chiropractor and more as a visionary, administrator and motivator of staff. A transformation must occur from the practice of Chiropractic to the business of Chiropractic. A shift in thought from "This is what I do, I will do more of it myself" to "I must delegate responsibilities to grow" is a necessity for growth. Your interest will increase in other related areas such as computer technology for higher efficiency, human resources for quality hiring and training, and improvements in basic business modus operandi. Your responsibilities will include investigating the best purchasing opportunity for printing, phone, x-ray sales and service, malpractice and business liability insurance and durable Chiropractic supplies. The doctor that puts their energy into implementing specific systems that can evaluate productivity and efficiency within their practice is working on their practice.

There are three interdependent traits I have found that define success in any project but definitely in multiple office expansion; vision, motivation and system implementation. The vision you create will be the motivating force that develops quality systems that can be highly executed.

You, the Chiropractor must be the visionary; the leader that has a burning desire to climb this mountain. Your employees must understand, agree and follow this vision. Ideally, the Chiropractor, staff and patients should all be aligned in the mission of the practices.

This vision must encompass satisfaction for all involved. Each person working with you must have a communal as well as a personal interest in reaching the goal. By serving the community, the practice, and oneself professionally and financially, solidifies the entire organization with the vision.

No one person can execute all of the needed tasks to build and sustain multiple practices; it is a true team effort. To acknowledge this statement exemplifies the need to motivate your team. Not all employees are motivated by the same compensation or by the same management style. My experience has found that evolving into a well defined corporate culture enables the leader to anticipate the needs of the team. High employee diversity leads to an increased difficulty to motivate and manage. Similar personalities are motivated with similar and predictable ethics, expectations and rewards.

As the organization grows the talent it takes to manage and motivate must increase equally. My basic theory to successfully manage staff is to clearly define what is expected from the employee, have them agree to that level of competency and then motivate and manage them to accomplish that expected level.

"Success is in the system" and, "the system is in the solution" are common phrases in my offices. We live by them and grow by them. The most famous illustration of the concept is McDonalds. With a personnel turnover of three hundred percent they rely on systems to accomplish a predictable standard of quality and service. Once the system had been established replicating the service to over 25,000 worldwide locations was history.

In many one doctor multiple staff practices the Chiropractor has the least knowledge on the systems within their practice; the office manager runs "the show". This doctor is reliant on that person to the point of being held hostage. The doctor that peels away each major task within their practice and develops a system to complete that task by others will be able to grow into multiple practices. That doctor must first master the task, document the process or system and then put it in stone for all to use. Each system must have a component that evaluates and monitors the effectiveness of that specific system for constant improvement. This allows the doctor to delegate with the ability to monitor. The result of this process is growth; growth by organization, not by individual.

The most difficult challenge is to commit to multiple practices, financially and emotionally. Truly commit, not go into the plan by stating, "I will open a second office and see how I do". Once committed you become the architect, designing the business plan with your talents. Drawing from your past experiences you will transition concept into reality.

Personally, growing from one practice to six has been one of my most challenging and satisfying professional experiences. I can relate it to mastering a video game. In a video game, at first each move you make you hit a landmine that brings you close to death. As you begin to understand this new environment you develop a plan or vision to win. It soon becomes evident that a motivated workforce with a plan or system will conquer your opponent. Further repetition of the needed tasks increases your ability to perform at a level of excellence. With all of these forces behind you to gain intellectual strength, political influence and financial independence, you crush the opposition. Unfortunately, life doesn't imitate video games exactly.

Chiropractic Education Poses New Threat to Public Health With New Proposal


The Council on Chiropractic Education, the monopoly accrediting authority for the chiropractic profession, is about to adopt new rules that could pose a real threat the well-being of millions of Americans. The CCE has decided to use the power granted it (under false pretenses) to turn the entire chiropractic profession into an unwieldy group of "not quite Medical Doctors" and loose on an unsuspecting public.

For over 115 years, chiropractic has been a drugless, non-surgical natural remedy for many ailments that haven't responded to traditional treatment. Over the last 30 years, CCE has sneaked its way into allopathic medicine so subtly that only traditional chiropractors have recognized the push. And, they've done it in the most unscrupulous way. By pretending to be the watch guard over chiropractic to ensure its graduates are fully trained and capable of safely serving the public, CCE has actually used that authority to wield an iron fist over anyone in the profession who dared oppose their agenda.

In 2001, the organization was so bold, it stripped the largest chiropractic school in the world of its accreditation status to mute its founder and stop the flow of new students that wanted to enter the profession because of what it was designed to do from its onset. Only a ruling by Federal judge, Charles Moye, that CCE would probably lose its case in Federal court kept it from succeeding.

Few people know (and CCE will NEVER tell) that the vast majority of chiropractic students attend a mere handful of chiropractic schools. Those few schools are those that embrace and teach the founding principles of the chiropractic profession. In other words, the consumers of chiropractic education have shown with their dollars, their time and their lives that THEY want to be involved in a drugless, non-surgical profession dedicated to serving Mankind in a natural way.

Everyone knows about the problem with student loan default rates among graduates of chiropractic school over the last 30 years. What we're seeing is the result of an education that is hardly relevant to the practice of chiropractic being financed by Federal student loans to thousands of students, some who can't even use much of the education they (and you, the American taxpayer) have paid for. Isn't 30 years of leading all health professions in student loan default rates sufficient evidence that CCE has not only failed in its missions to ensure quality chiropractic education but to also protect the American public AND taxpayer?

Now on top of quadrupling the cost of and education that serves neither the profession nor the public, the Council on Chiropractic Education has decided that it should literally remove the essence of chiropractic principles from the very profession it claims to protect. In its proposed revised guidelines, CCE wants to remove the concept of nerve interference and any reference to "drugless, non-surgical" from its lexicon. Obviously, the intention is to begin re-shape the profession into something NO chiropractor ever imagined they'd be or ANY chiropractic patient would ever use. CCE would have chiropractors diagnosing medical condition, prescribing drugs and doing minor surgery.

CCE has accomplished much of its dirty work with subtle (?) nods from both the Federation of Chiropractic Licensing Boards (FCLB) and national Board of Chiropractic Examiners (NBCE) as informal co-conspirators in this dangerous veer toward Medicine.

I love being a chiropractor. I chose to be a chiropractor and NOT a Medical Doctor. Their job is difficult enough. Imagine if you will one day having 60,000 chiropractors turned loose thinking and serving as "not quite" MDs. Some might do well. Honestly, there are probably a few chiropractors that preferred to be MDs that just could not get in to medical school that would still make great MDs. But, imagine the tens of thousands that would eventually be granted those privileges that should not have those privileges and could not handle them if granted.

The Council on Chiropractic Education with its proposed changes threatens both the chiropractic profession and American public. Drastic measures should be taken by the profession, obviously. But, the public should get involved and contact their Congressman and Senators. They should also contact the US Dept. of Education to complain about CCE and encourage DOE to remove CCE's status and either replace it with one without an agenda or, at least, allow another agency to offer an alternative to those schools that choose to offer chiropractic education consistent with both chiropractic principles and the consumers who demand THAT education.

Acupuncture and chiropractic care are known to have many benefits. Most people believe these benefits only pertain to pain in the body, especially in the spine, leg, and neck regions. Overuse Injuries - What Causes Them and How to Avoid Them - Part 2


Overuse or repetitive motion injuries are the result of repetitive forces that overtime create so much stress that the tissues finally fail partially or completely. This type of injury is different than sports injuries that are the result of an acute or sudden force that is too much for the tissue to overcome (e.g. a sprained ankle or broken bone). Most parents, coaches and athletes do not readily understand overuse injuries or know how to prevent them from occurring.

As we discussed in Part 1 of this series we mentioned there are three major causes of overuse injuries, particularly in running. The last article discussed increased training load or volume as the first cause of these types of injuries and what a runner should do to prevent this type of injury. In this article we will discuss inadequate warm-up as cause of overuse injury.

A muscle that is not warmed up is less elastic and therefore when put under greater forces like doing tempo or speed work will tear more easily or result in more forces to the ligaments causing them to tear (sprains/strains and tendonitis injuries). Many individuals especially runners have been falsely lead to believe that stretching before they run will help to prevent injury. Unfortunately, the science on this practice is not convincing. Furthermore, any time you attempt to stretch or elongate a muscle when it is not warmed up you run the risk of tearing or injuring the tissue.

So, how should one go about warming up for running or any other sport for that matter if stretching is not the answer. It is not that stretching is bad, it is just that if done to aggressively before there is adequate blood flow in muscle it can lead to injury. Regardless of the sport all activities should begin with lower intensity movements that help to increase blood flow to all muscles that will be in potential use. Easy jogging or cycling for at least 5-10 minutes serves as a good warm-up for almost any sport and prepares the body for more intense running. Vigorous stretching should not occur until after some light warm-up has taken place. I recommend very little stretching if any at all until you have run for at least 5 minutes.

The use of a medicine ball or foam roll to lightly and rapidly roll over the muscles to be used helps to increase blood flow and serves as a good warm-up prior to activity. Once you have completed foam rolling and then 5-10 minutes of easy jogging or shadowing the movements of the activity you are going to be training, then it is okay to stretch the major muscles involved. However, even now the purpose of the stretching prior to begin the intense phase of running or any other activity is to prepare the muscle for that activity, as opposed to trying to improve overall flexibility. So, if one were going to do speed work, after the brief jogging period performing some high knee running, strides and butt kicks will help to move the muscles into a greater range of motion, just like a stretch does, and prepares it for the faster running that is about to occur. If you were going to be sprinting or hurdling the use of ballistic or bouncing type stretches would actually be most appropriate for the activity, provided you have gotten some blood to the muscle first.

So, warming up properly usually does not start with stretching and ideally uses an activity that will bring blood to the area first and then stretch the muscles or attempt to mimic the activity to be performed. Once your are finished with your activity this is actually the best time to attempt to stretch for improving overall flexibility. At the end of your run the muscles are warm and full of blood and can be stretched further than at any other time.

DISC TEAR

This type of disc problem refers to a crack or micro tear in the tough outer ring of the disc. This may lead to fluid leaking from the disc and causing it to wear thin.

BULGING DISC

Occurs when the soft jelly-like material in the middle of the disc pushes to one side -forwards or backwards - causing swelling. This swelling can contribute to pressure on the spinal nerves triggering off painful symptoms.

HERNIATED DISC

When a disc becomes herniated, the fluid from the centre of the disc ruptures through the outer fibres, stretching the disc beyond its normal limits.

DISC PROLAPSE

Refers to a condition in which part of the disc tears away and becomes a free floating fragment.

DESSICATED DISC

Occurs when the disc loses its fluid content, and degenerates down to a rough, worn down appearance. This occurs as the bones begin to fuse to each other.

What are some symptoms I may experience as a result of a disc condition?

Symptoms of a disc problem may vary with each individual person. For some, a disc condition may cause little to no pain. For others, a disc problem can push onto nerves in the spine, resulting in a lot of pain, and limited movement. Some symptoms you may experience as a result of this includes, a dull or sharp pain, muscle spasm or cramping, weakness, tingling or referred pain (pain into other parts of the body). It could even make standing, sitting, walking, lifting, urinating, defecating, sneezing, coughing, and moving nearly impossible. In extreme cases, numbness of the legs or foot, or a loss of muscle control may occur.

What causes disc problems?

While pain due to a disc problem may come on all of a sudden, it is usually the result of wear and tear over a long period of time.

For instance, a middle-aged accountant who sits for long periods of time suffers from disc dehydration, and has poor posture and spinal mobility, then lifts a heavy piece of furniture and experiences acute back pain and leg pain.

Or perhaps many months of repetitive lifting, bending or twisting, he experiences no pain at all, until he sneezes. Bingo! A herniated disc.

We mentioned earlier that a lack of spinal mobility may contribute to problems with your discs and may hence contribute to back pain.
